Apply now and achieve your professional goals with a team that honors and protects the lives of individuals by supporting choice through advocacy, education, and facilitation of services!DESCRIPTION OF WORKIn this position, you will provide care management services to older adults in need of protective services by developing care plans, identifying resources, and assessing the level of care needed to effectively support each person. Your work will involve obtaining information to identify social, economic, emotional, health, or physical needs of our consumers and assisting them in obtaining a variety of services. This includes developing, implementing, monitoring, and modifying person centered care plans to support older adults, as well as working in close cooperation with other social agencies, hospitals, clinics, courts, and community resources in planning to meet the needs of consumers and assisting the consumer in utilizing resources. • Full-time employment
• Work hours are 8:00 AM to 4:00 PM, Monday – Friday, with a 60-minute unpaid lunch.
• Telework: You will not have the option to telework in this position.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITYQUALIFICATIONSMinimum Experience and Training Requirements:

• Six months as an Aging Care Manager 1 or a County Caseworker 1; or
• Successful completion of the County Social Casework Intern program; or
• Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training.

• This position falls under the provisions of the Older Adult Protective Services Act.
