Location: Franciscan Care Center – Sylvania, OhioRooted in the rich Franciscan tradition of compassionate, quality care, Franciscan Care Center is a residential and rehabilitation community dedicated to serving older adults and those in need of extensive rehabilitation services. We are currently seeking a Licensed Social Worker (LSW) to serve as our Director of Social Services.About the Role:
The Director of Social Services plays a vital role in supporting the emotional and psychosocial well-being of our residents and their families. This leader collaborates closely with the interdisciplinary team to ensure that care plans are person-centered and promote dignity, independence, and quality of life.This position requires an active LSW (Licensed Social Worker) license in the state of Ohio.Key Responsibilities:Conduct pre-admission interviews with prospective residents/guarantors and explain center services.Partner with the Director of Nursing to evaluate patients prior to admission.Facilitate the admission process and document all forms and agreements.Assess resident needs and develop individualized social service plans.Provide emotional support to residents and families, including crisis intervention.Maintain relationships with outside agencies to coordinate resources and services.Plan and deliver staff in-services on the social aspects of long-term care.Coordinate discharge planning and continuity of care services.Maintain accurate and confidential social service documentation and resident care records.Participate in care plan conferences and departmental meetings.Engage with the community through outreach and professional development activities.Exceptional Benefits Package:We’re proud to offer benefits that reflect how much we value our team:Health & Wellness: Medical, Dental, Vision InsuranceFinancial Security: Employer-funded HSA, 401(k) with generous employer contributionsCareer Growth: Tuition reimbursementTime Off: Paid Time Off (PTO) with cash-out optionSupport Services: Company-paid life and disability insurance, EAPWage Access: Access earned pay before payday via PayActiveRecognition: Annual merit increasesMake a Difference Every Day
